We are a partnership of health and care organisations that come together to plan and deliver joined-up services and to improve the health and wellbeing of people who live in Kirklees. Our focus is on preventing ill health, improving health and healthcare, reducing health inequalities, making the best of our collective resources, and putting communities at the heart of all we do.
Our partnership brings together the local NHS, other health and care providers, Kirklees Council, Healthwatch, and community and voluntary sector organisations, to arrange and deliver services for the 470,00 people who live in Kirklees.
We support the delivery of the Kirklees Health and Wellbeing plan (pdf) and are committed to achieving the following four aims:
- improve outcomes in population health and healthcare
- tackle inequalities in outcomes, experience and access
- enhance productivity and value for money
- help the NHS support broader social and economic development
