The Leeds Health and Care Partnership (LHCP) includes health and care organisations from across Leeds who are working together to improve the health of people in Leeds. We understand that by listening to people, and by sharing our knowledge and resources, we are able to make a bigger difference to people’s lives.

We are part of the wider West Yorkshire Health and Care Partnership which is an 'Integrated Care System' working to improve the health and wellbeing of people across West Yorkshire.
